Please be advised that I am retiring from my Dermatology practice effective November 25, 2026.
Dr. Karla Pivik of Juniper Dermatology, LLC in Bend will be taking over my practice. I am confident that you will be in excellent hands with Dr. Pivik and her dedicated team for your ongoing dermatologic care.
Your medical records will remain at my current practice address until November 24, 2026, and after that they will be maintained by Dr. Pivik at Juniper Dermatology:

If you would prefer to have your records transferred to another provider, please submit a written request form to my office, and we will be happy to arrange a transfer. My office will remain available here to request records transfer until January 25, 2027. After November
25, 2026 you may also choose to contact Dr. Pivik’s office to request your records be transferred to another provider.
